Cover:
- a book of blue or red color;
- size: 142х233 mm (± 0.5 mm);
- blind embossing with the State Emblem of Ukraine.
Spread:
- series (two letters), 6-digit number;
- graphic images and images for protection purposes;
- information on the obtained qualification;
- signature of the official and seal;
- date of issue.
The left and right sides of the form contain typed textual information in accordance with the template in Ukrainian and English.
