- series and registration number of the diploma assigned during registration in the Unified State Electronic Database on Education;
- position and signature of the head (authorized person);
- seal;
- a form of at least A4 size (210х297 mm);
- is filled out in two languages (Ukrainian and English);
- own template (form, design, printing, paper parameters, etc.).
(!) Information and data on Doctor of Arts Diplomas issued starting from 2022 are available in the Register of the Unified State Electronic Database on Education.
The documents issued by higher education institutions of Ukraine are not limited to the examples posted on the ENIC Ukraine website.
You can check the documents in the register of documents on higher education on the website of Inforesurs, State Enterprise, or check the apostille stamp of the Ministry of Education and Science on this website. If the check is unsuccessful, you should send a request to the higher education institution or our Centre.

Example of a certificate on awarded (advanced) vocational qualification starting in 2015
Cover:
- a book of blue or red color;
- size: 142х233 mm (± 0.5 mm);
- blind embossing with the State Emblem of Ukraine.
Spread:
- series (two letters), 6-digit number;
- graphic images and images for protection purposes;
- information on the obtained qualification;
- signature of the official and seal;
- date of issue.
The left and right sides of the form contain typed textual information in accordance with the template in Ukrainian and English.
Example of a Qualified Worker’s Diploma of 2015-2024
Cover:
