Qualified worker diploma has uniform requirements for execution and is drawing up on the state-approved form, according to the Resolution of the Cabinet of Ministers of Ukraine No 645 "On Vocational (Vocational-Technical) Education Documents of State Standard and Supplements thereto», dated July 22, 2015»

Form:

• paper document with the size 148 х 210 mm (± 0,5 mm);
• watermarks over the entire area of the document.

Front side:

• image of the small State Emblem on the left side of the form;
• the inscription «УКРАЇНА» on the left side of the form, the inscription «ДИПЛОМ кваліфікованого робітника» under it;
• series (one letter and two digits), 6-digit number;
• signature of the official and seal;
• date of issue.

Reverse side:

• series (two Cyrillic letters), 6-digit number are placed at the bottom of the form on the right.
