EDUCATIONAL DOCUMENTS IN UKRAINE

This section contains collected samples of educational documents with specified obtained in Ukraine qualifications, which are recognized in the formal education system. The samples are organized by years, covering the period from 1961 to the present.
A single model and unified forms of documents on education were approved AND used by all educational and research institutions until September 2014.
Nowadays,
- documents on general secondary education, as well as vocational (vocational-technical) education and training are issued according to templates approved by the Ministry of Education and Science;
- documents on professional pre-higher education - in accordance with the requirements for the forms and content of documents on professional pre-higher education, approved by the Ministry of Education and Science;
- documents on higher education - in accordance with the forms of documents on higher education (research degrees) and supplement to them, a template of transcript of records, approved by the Ministry of Education and Science.
Professional pre-higher and higher education institutions have the right to issue documents on education on the forms of their own design.
Information about documents on education shall be entered into the Unified State Electronic Database on Education (USEDE).
Registration of higher education documents issued by higher military education institutions (higher education institutions with specific training conditions) is carried out in the USEDE in accordance with the procedure established by law.
Please note that the transitional period applied to updated templates of educational documents lasted for a certain period of time. During this period, educational institutions could draw up and issue documents both on the forms of the previous and the current template.
Transitional periods in the forms of educational documents:
1) from 1991 to 1993 - issuance of all educational documents on forms with both Soviet and Ukrainian symbols;
2) from 1997 to 2000 - issuance of all educational documents in both paper and plastic form;
3) from 2014 to 2015 - issuance of documents on higher education, both on the form of the model of previous years, and on the forms approved by the HEI.
