Since May 2015, there has been no single form for transcripts of records.

Transcripts of records of different higher education institutions may have differences in appearance (material, design, placement of information, color). The sample form of the transcript of records and (if necessary) the protection technology are approved by the decision of the academic (pedagogical) council of the education institution.

Example of 2015-2016

• a form of at least A4 size (210x297 mm);
• is filled out in Ukrainian;
• positions and signatures of officials;
• seal.

Example of 2016-2020

Since June 2016, information has been provided in Ukrainian and English.

• positions and signatures of officials, their surnames and initials;
• seal.

Example of 2021

• is filled out in Ukrainian and English;
• the block of information on academic mobility and the scientific component is included in the transcript of records, if necessary, if the transcript is formed based on the results of academic mobility or for educational and research programs;
• position and signature of the official, his/her name and surname;
• seal.

Example starting in 2021 (the third-cycle higher education programs)

Examples of forms of educational documents issued by higher education institutions of Ukraine are not limited to the examples posted on the ENIC Ukraine website.

You can check the documents in the register of documents on higher education on the website of Inforesurs, State Enterprise, or check the apostille stamp of the Ministry of Education and Science on this website. If the check is unsuccessful, you should send a request to the higher education institution or our Centre.
