Form:

- the small State Emblem on the left;
- the inscription “UKRAINE” and the inscription “Junior Specialist’s Diploma” below it on the left;
- series (one letter and two digits), 6-digit number (assigned by the Unified State Electronic Database on Education);
- information on the obtained qualification;
- signature of the official;
- seal;
- date of issue.
The back:
- series (two Cyrillic letters), 6-digit number are placed at the bottom of the form on the right.
